Subungual Hematoma Treatment in Melaka
Subungual Hematoma
A subungual hematoma, commonly known as blood trapped under the nail, can cause significant discomfort and pain. At Klinik Liysha, our experienced team provides prompt and effective treatment to alleviate symptoms, prevent complications, and restore the health and appearance of your nails.

What is a Subungual Hematoma?
A subungual hematoma can be referred to using several other terms, including blood under the nail, a nail bruise, subungual bruise, nailbed hematoma, etc.
A subungual hematoma occurs when blood pools under the nail, often as a result of trauma, such as stubbing a toe, dropping a heavy object on the finger, or repetitive pressure from tight footwear. It typically affects toenails and fingernails, leading to discoloration, pain, and swelling.
Symptoms of Subungual Hematoma
Discoloration: A red, purple, or black spot develops beneath the nail. The intensity and extent of discoloration may increase as more blood accumulates.
Throbbing Pain: The pain is typically sharp or throbbing and is caused by the pressure from the trapped blood. Pain intensity often correlates with the amount of blood pooled under the nail and the degree of swelling.
Swelling: Swelling is usually localized to the fingertip or toe affected by the injury. The skin around the nail may appear puffy, warm to the touch, and sensitive.
Nail Detachment: In some cases, the nail may feel loose or lifted soon after the injury. Over time, the damaged nail may separate partially or entirely from the nail bed.
Draining Subungual Hematoma
Nail Trephination
A safe, minimally invasive procedure where a small hole is created in the nail to drain the trapped blood. This technique provides immediate pain relief, preserves the integrity of the nail and requires minimal recovery time.
For severe cases where the nail is significantly damaged or detached, we may recommend removing part or all of the nail to allow proper healing.

What to Expect During Your Appointment
Preparation
The doctor evaluates the severity of the hematoma and cleans the affected area. Local anesthesia may be used for comfort if necessary.
Creating the Hole
A small hole is made in the nail using a heated needle, manual drill, or laser to allow the trapped blood to drain out.
Drainage
Once the hole is created, the trapped blood is allowed to drain naturally. Gentle pressure may be applied to encourage complete drainage.
Post-Procedure Care
The area is cleaned, a bandage is applied, and pain management advice is given. The nail will heal and regenerate over time, with a follow-up if needed.

FAQ on Nail Trephination
Frequently Asked Questions
It is performed to alleviate the pain and pressure caused by a subungual hematoma (blood under the nail). Without drainage, the pressure can cause significant discomfort and potentially damage the nail.
Most patients experience minimal discomfort during the procedure, especially since the area is usually numbed with local anesthesia. Afterward, some mild pain or tenderness may occur, which can be managed with over-the-counter pain relievers.
Recovery is typically quick. The small hole in the nail will heal as the nail grows out over the next few weeks. You may experience some tenderness for a few days, but it should subside quickly.
Yes, the hole will gradually close as the nail grows. The nail itself will continue to regenerate, and any remaining discoloration or blood underneath will gradually disappear.
While nail trephination is generally safe, there are minor risks, including infection, continued pain, or nail damage if the procedure isn’t done properly. However, these complications are rare when performed by a trained professional.
